How I’d Split Your Acne Routine as a Physician: Skincare, Diet & Lifestyle

If you’ve been fighting adult acne with product after product and still not seeing the results you want, you might be missing two-thirds of the equation.
As a board-certified physician, I look at acne-prone skin differently than a lot of routines out there. Most people treat acne like it’s 100% a skincare problem — cleanser, serum, moisturizer, repeat. But in my experience, a truly effective acne routine breaks down more like this:
Skincare (the largest piece, but not the only piece)
Skincare is still doing the heavy lifting — around half of the equation. This is where consistency matters most: a gentle cleanser, a targeted treatment step, and a moisturizer that supports your skin barrier instead of stripping it. For a lot of my clients with acne-prone skin, this is where a salicylic acid toner and an oil-free moisturizer come in — salicylic acid helps keep pores clear, while a lightweight, non-comedogenic moisturizer keeps skin from overcompensating with extra oil.
But skincare alone can only take you so far if the other two pieces are ignored.
Lifestyle
Stress, sleep, sweat, and even the pillowcases and phone screens touching your face daily all play a role in breakouts. Things I talk to patients about constantly:
Sleep quality and stress levels (both affect hormone patterns tied to breakouts)
Sweat left on skin post-workout
Fabric and product buildup on pillowcases, phones, and masks
Hands-on-face habits
None of these replace skincare, but ignoring them means your skincare routine is working against a moving target.
Diet
This is the smallest slice, but it’s not nothing. Diet doesn’t cause acne on its own, and no food is guaranteed to trigger a breakout for everyone. But there is a real, if individual, link between certain high-glycemic foods, dairy, and breakout patterns for some people. I don’t recommend cutting out entire food groups — I recommend paying attention to your own patterns and adjusting from there.
The takeaway
If you’ve only ever focused on skincare, you’re not doing anything wrong — you’re just missing part of the picture. A more complete acne routine looks at all three: what you’re putting on your skin, how you’re living day to day, and what you’re eating. Small, sustainable changes across all three tend to outperform an aggressive skincare routine alone.
